Target Audience
This activity is intended for Physicians, Physician Assistants, Nurse Practitioners, Registered Nurses, Social Workers, Case Managers, and Resident Care Facility/Nursing Home Administrators engaged in Care Transitions and Readmission Prevention of patients.
Educational Objectives
After completing this activity, the participant should be better able to:
Describe the core skills necessary to construct a comprehensive bundled payment program that focusses on quality care delivery from hospital to nursing facility to home, in a cost efficient manner.
Effectively communicate transitioning patients in levels of care from hospital to skilled nursing facility to home environment.
Discuss the advantages and utility of community-based programs available at Federal and State levels to assist in the efficient transition to value-based models.
